Day 3: First Wash — How to Do It Safely
Day 3 is typically first-wash day — a milestone that makes most patients nervous. The rule: no direct shower stream, no rubbing. Lather a mild or clinic-provided shampoo into foam in your hands, pat it gently onto the recipient area, rinse with low-pressure lukewarm water (a cup works well), and air-dry or pat with paper towel.
What to expect
- Swelling starting to move down and out (often past its peak)
- Scabs that survive the first wash — that’s expected, they’re not supposed to come off yet
- Relief: the first wash removes dried blood and reduces itching
How to care for it
- Follow your clinic’s exact wash protocol — frequency, products, and technique vary
- Water pressure: pour, don’t spray. Lukewarm, never hot
- Do NOT pick at scabs during or after washing

What shampoo should I use after a hair transplant?
Whatever your clinic gave or specified — typically a mild, fragrance-free baby shampoo or a clinic-branded foam. Avoid anything with harsh sulfates, menthol, or exfoliants for the first couple of weeks.
