Days 15–21 (Week 3): Shedding Ramps Up, Restrictions Lift
Week 3 is when shock loss stops being theoretical: transplanted shafts come away in the shower and on your pillow, and the recipient area visibly thins. It looks like going backwards; it isn’t — the follicles stay implanted while their shafts cycle out. Meanwhile normal life returns: most clinics have cleared exercise, hats, and gentle regular hair washing by now.
What to expect
- Noticeable daily shedding of transplanted hairs — often the most emotionally jarring week
- Residual redness or pinkness in the recipient area, especially on fair skin
- Donor area mostly comfortable, with occasional itching or pimple-like spots as hair regrows through healed skin
- A return to routine: work, gym, hats, and normal sleeping positions per your clinic’s clearance
How to care for it
- Wash normally but gently — shedding shafts in the drain are expected, and washing does not cause graft loss at this stage
- Keep up sun protection: a loose hat or SPF once your clinic approves sunscreen on the recipient area
- Resume cardio and weights if your clinic has cleared them (most do between days 10 and 14)
- Take your week-3 photo set — this is the "before" for the regrowth story ahead

Can I wear a hat 3 weeks after a hair transplant?
